Resources and Important Forms

Depending on the type of disciplinary process a student is engaged in, they may need to access and complete certain forms. All forms should be reviewed and completed by the individual student using the links below:

  • University Reporting Landing Page - This is the centralized landing page for all incident reporting forms on WSSU’s campus.
  • Appeal Form- This form allows students to appeal the outcome of any disciplinary process facilitated by the Office of Community Standards & Civility and/or Housing & Residence. If you are looking for the Housing & Residence Life appeal form or the Title IX appeal form, please go to the University Reporting Landing Page for those forms.
  • Acceptance of Responsibility (AOR) Form - This form is to be completed if you are accepting the charges brought to in your administrative hearing.
  • Disciplinary Standing Request Form - This form is for faculty, staff and/or student to have a copy of their student disciplinary records or have them released to third parties by the Office of Community Standards & Civility.
  • Student Conduct Experience Evaluation - This form is to collect feedback regarding the student conduct process.
  • Notice of Representation - This form is for a licensed attorney or another advocate to participate in the hearing process, a student or student organization must provide a completed form to the Office of Community Standards & Civility.
  • Rule 700.4 Lawyer Involvement Stipulations - This form provides information regarding how the constituent institutions of The University of North Carolina System will interpret and administer the requirements of North Carolina General Statutes Section 116-40.11 regarding the participation of counsel and non-attorney advocates on behalf of students and Student Organizations in disciplinary proceedings.
  • Requirements and Guidelines for Licensed Attorneys/Advisors for Disciplinary Proceedings -This form provides guidelines, rules, and regulations that must be followed by all attorneys and/or advocates during the disciplinary process.
  • Student Conduct Council Training (coming soon)
  • Students’ Code of Conduct - This is the most recent version of the Students' Code of Conduct.
  • Institutional Policies - This is a listing of all WSSU institutional policies and regulations.
  • Submit Restorative Justice Sanctions - This form may be used by students or student organizations to submit sanctions imposed for violating the Students’ Conduct or other associated policies.
  • Release of Academic/Disciplinary Information (FERPA Form) - This form allows the Office of Community Standards & Civility to release information to identify individuals, such as a student’s parent, legal attorney, etc.
  • Advisor means a Civility Advisor, Attorney, Non-Attorney Advisor, or Observer, as those terms are defined below.

  • Civility Advisor means a faculty, staff, or student member who has received training through the Office of Community Standards & Civility and has knowledge of the conduct process. A Civility Advisor is not an attorney. The Civility Advisor provides advice or consultation to a student who has been charged with a violation under this Code or to a student bringing forth information. While the Respondent and Complainant are each responsible for explaining their own account of the events at issue, the Civility Advisor serves solely in an advisory capacity and may not act in place of the Complainant or Respondent.

  • Attorney means any person licensed to practice law in the State of North Carolina.

  • Non-Attorney Advisor means any person who is not licensed to practice law in North Carolina and whom a student selects to assist them in the conduct process.

  • Observer means any person, other than an Advisor, whom a student selects to serve in a non-speaking, non-participatory capacity. An Observer may not participate as an Advisor or witness in the conduct process.

  • Controlled Substance means any drug, substance, or immediate precursor listed in Schedules I through VI of the North Carolina Controlled Substances Act, N.C. Gen. Stat. §§ 90-86 through 90-113.8, including but not limited to marijuana, cocaine, heroin, methamphetamine, fentanyl, MDMA, and LSD, as well as any prescription drug possessed or used without a valid prescription.

  • Disciplinary or Conduct Procedure(s) means a hearing or other procedure during which a designated University official, board, or panel considers information to determine whether a student or student organization may have violated this Code.

  • Good Disciplinary Standing is defined as a student enrolled at the University, or a Student Organization registered with the University, that: (1) has no findings of responsibility for conduct violations; or (2) has satisfied all imposed sanctions and is no longer subject to any active disciplinary status, including disciplinary warning, probation, deferred suspension, suspension, loss of privileges, or any other conditions or restrictions imposed as a result of a conduct proceeding.

  • Student means any of the following:
    • a person who has applied for admission to and been accepted into any academic program of study at the University;
    • a person enrolled in or attending classes at or through the University, whether at the University's main campus or at any remote location, including a person enrolled in online or distance education programs or a person participating in a Study Abroad program; or
    • a person participating in any pre-matriculation orientation program.
  • University-Affiliated Student Organization (“Student Organization”) means a student group that has been officially recognized or sponsored by Winston-Salem State University in accordance with the University's student organization recognition policies.
  • University Premises means all buildings, facilities, structures, grounds, and property owned, leased, operated, controlled, or supervised by Winston-Salem State University, whether located on or off the main campus, including but not limited to academic buildings, administrative offices, residence halls, dining facilities, athletic and recreational facilities, parking lots and structures, sidewalks, open grounds, and any other real property in which the University holds an ownership, leasehold, licensing, or other possessory interest. University Premises also includes any vehicles owned, leased, or operated by the University, as well as any property temporarily under the University's custody or control for purposes of a University-sanctioned function or event.
  • University-Sponsored Activity means any event, program, activity, or function that is initiated, authorized, sanctioned, funded, or supervised by the University, a University department or office, or a recognized student organization, regardless of whether the activity occurs on University Premises or at an off-campus location. University-Sponsored Activities include, but are not limited to, athletic events, academic programs, field trips, study abroad programs, conferences, student organization events, internships conducted under University auspices, and any other activity conducted in the name of or on behalf of Winston-Salem State University.
