Reporting

Winston-Salem State University encourages the campus community to report concerns directly to a supervisor or manager. If you don’t feel comfortable raising a concern with your supervisor or manager (or if your concern relates to them, or if you have previously told them but the concern has not been remedied), there are several ways you can report your concern:

  • Report the issue to your supervisor/manager’s direct manager
  • Report the issue through one of the specialized reporting avenues at WSSU

Clery Act Reporting

Campus Security Authority (CSA) Incident Reporting – the University Police Department encourages all members of the campus community to report crimes on a timely basis. However, under the Clery Act, CSAs are required to report Clery Act qualifying crimes that occurred on campus, in public areas near campus, and in certain non-campus buildings owned or controlled (leased) by the University. Fraud, Waste & Abuse of State or University Resources

The Internal Audit reviews violations of state or federal law, University rules or regulations, fraud, misappropriation of state or University resources, and/or substantial and specific danger to the well-being of the University community. Office of Internal Audit
Information Security Incidents If you suspect a computer abuse or security incident, please inform the WSSU OIT Information Security Officer using either the online or paper-based submission methods below. The incident will be directed to the Information Security Services Team. A preliminary analysis of the incident will take place by the team who will determine whether Incident Response Team activation is appropriate. Office of Information Technology
Incident Reporting

The purpose of the Incident Reporting Form is for students, faculty, staff, and non-WSSU individuals a way to submit issues and instances of concerning behavior to the Office of the Dean of Students. We encourage using this form when students or other campus community members exhibit behaviors that need to be addressed.

Occupational Accidents & Illness

Notify your supervisor if you receive a work-related injury, even if you do not require medical treatment. The supervisor has the responsibility of ensuring that the employee receives medical treatment immediately and notifying the university Workers' Compensation Administrator(s) or designee as soon as possible in order to begin the process of administering the claim. Student Title IX/Sexual Misconduct Any member of the Winston Salem State University community, including visitors and third-party vendors, can use this form to report incidents of sexual misconduct that occur on or off campus involving students, faculty, staff, and third-party vendors.
