How to Report an Information Security Incident

If you suspect a computer abuse or security incident, please inform the WSSU OIT Information Security Officer using either the online or paper-based submission methods below. The incident will be directed to the Information Security Services Team. A preliminary analysis of the incident will take place by the team who will determine whether Incident Response Team activation is appropriate.

Your reported incident and your contact information will remain confidential. Our team takes anonymous tips and incidents as well, but please be aware that without a person to contact an investigation may not be possible or effective.

Online Incident Report Submission

  1. To submit an IS incident anonymously via the online form, you may leave the contact information fields blank.
  2. Complete the details and click ‘Submit.’

Paper Incident Report Submission

Complete the Information Security Incident Report form then submit by one of the following methods:

  • Email to
  • Fax to 336-750-2093
  • Send via inter-office mail to "OIT, EJCS Building – #1209"
